Let’s talk about morphine dose changes.
Yes, yes, if you didn’t know, different routes of morphine require different doses. 10mg oral morphine IS NOT the same as 10mg SC morphine. How to quickly convert? here are the basics.
Oral morphine mg/day —> SC morphine mg/day = DIVIDE by 2 (multiply by 2 for vice versa)
Oral morphine mg/day —> SC diamorphine mg/day = DIVIDE by 3 (multiply by 3 for vice versa)
A 12mcg transdermal fentanyl patch = approx. 30mg ORAL morphine.
QUICK EXAMPLES: Random numbers, not medically accurate
A patient that takes 10mg oral morphine each day is about to be switched to SC morphine. How much should he be given per day? 10mg/day / 2 = 5mg/day of SC morphine
A Patient taking 40mg of SC morphine per day to be switched to oral morphine. How much per day? 40mg x 2 = 80mg/day oral morphine
A Patient on 90mg/day SC diamorphine wants to move to oral morphine, how much per day? 90 x 3 = 270mg oral morphine/day
